    Here, you'll find all the necessary forms and information required for your student to participate in our athletics programs. Our goal is to make the registration and participation process as smooth and straightforward as possible for both students and parents.

    Sports Physicals
    A current sports physical is a mandatory requirement for participation in any FSPS athletic program. This ensures the health and safety of our student-athletes. If you don't have a primary care provider, call the School-Based Health Center to get a physical today!

    We've partnered with RankOne to streamline our forms process. Follow these steps to complete the necessary forms online:

    1. Visit RankOne Website: Go to www.rankone.com and select “PARENTS CLICK HERE!”

    2. Access Forms: Choose “GO TO FORMS” and select your State and the Fort Smith School District.

    3. Create a Parent Account: Follow the on-screen instructions to create a new account or log in if you already have one. For detailed guidance, see our "How to Create a Parent Account Tutorial" below.

    4. Link Your Student: Once logged in, link your student to your account using the required information.

    5. Complete and Submit Forms: Navigate to the forms section and complete all required electronic documents.

    Creating a parent account on RankOne is easy and allows you to manage your student's athletic forms in one place. Here's a quick guide:

    1. Register: Visit the RankOne website and click on “Create New Account” or use your Facebook/Google Plus credentials.

    2. Verify Email: Check your email for a confirmation link and click on it to verify your account.

    3. Sign In: Use your registered email and password to sign in.

    4. Link Your Student: Add your student’s details to link them to your account.

    5. Start Completing Forms: Select “Start Forms” and fill out the required electronic documents.
